This program is facilitated by SWAMP Central Coast and funded by Central Coast Council.
OBJECTIVE
The Sustainable Wetlands Agricultural Maker’s Project (SWAMP) together with You Connect facilitate a nature, gardening, cultural and educational program for the participants of You Connect.
This collaborative program is beneficial to the wellbeing of our participants and members with physical and mental health, nutritional health and connection to the greater community. It is an empowering and therapeutic experience for all, allowing growth, understanding and connections to flourish.
OUTCOME
Positive impacts with their speech, social skills, everyday skills, attention to details, creativity, enjoyment, connection to community, cultural awareness, and physical and mental health.
PROGRAM OUTLINE
• Natural Art Therapy
• Connection to Land and Culture
• Permaculture
• Life on a Farm
• Eco-friendliness
• Cycle of Food from Paddock to Plate
• Connecting with the community including local farmers, nurseries, local parks, cultural sites etc

Activity Cost $5/ week

By booking into the Connecting Back to Nature Program, I agree You Connect will charge the NDIS 9 - 12pm at your agreed ratio.
I acknowledge that the cost of activities is not covered by the NDIS and I will pre pay now.
I acknowledge that the cost of transport is charged on top of supports.
As per the NDIS Pricing Arrangements and Price Limits, You-Connect will charge 100% of the agreed fee associated with the activity from your plan where you have provided a short notice cancellation (or a no show).
A cancellation is a short notice cancellation if the participant:
· Does not show up for a scheduled support within a reasonable time, or is not present at the agreed place and within a reasonable time when the provider is travelling to deliver the support; or
· Has given less than seven (7) clear days’ notice for a support
Where a participant will not be available to receive support for a period of more than 5 days (e.g supported holiday, family holiday) You-Connect requests that a minimum of 4 week's notification is provided. Failure to notify You-Connect of an extended absence will result in You-Connect making claims for payment of schedule of scheduled supports to meet industrial relations obligations to staff.
