By booking into the Easter Holiday Program, I agree You Connect will charge the NDIS 9am - 3pm at your agreed ratio

I acknowledge that the cost of activities is not covered by the NDIS and I will pre pay now.

As per the NDIS Pricing Arrangements and Price Limits, You-Connect will charge 100% of the agreed fee associated with the activity from your plan where you have provided a short notice cancellation (or a no show).

A cancellation is a short notice cancellation if the participant:

· Does not show up for a scheduled support within a reasonable time, or is not present at the agreed place and within a reasonable time when the provider is travelling to deliver the support; or

· Has given less than seven (7) clear days’ notice for a support

Where a participant will not be available to receive support for a period of more than 5 days (e.g supported holiday, family holiday) You-Connect requests that a minimum of 4 week's notification is provided. Failure to notify You-Connect of an extended absence will result in You-Connect making claims for payment of schedule of scheduled supports to meet industrial relations obligations to staff.

CAVES BEACH + LUNCH AND FISHING AT MURRAYS BEACH AND/OR SWANSEA

9:00am Meet at You Connect

9:30am – Depart for the activity.

10:00am – Explore Caves Beach.

12.00pm- Head to Catherine Hill Bay for lunch, beach walk and photography of the wharf

1.00pm – Fishing in Swansea/Murrays Beach (location will be picked based on weather and suitability)

2:00pm- Depart Newcastle and head back to Centre.

2:45pm - Arrive back at Warabrook ready for pick-ups and afternoon transports.

 

 

*Requirements: Bring your own morning and afternoon tea, lunch, drink, hat, sunscreen and concession card (if you have one). A towel is recommended in case of wet/sandy feet.
